  3. Recipient information: Details about the person who received the documents.

  4. Description of the documents: What specific documents were served.


The affidavit is then filed with the court to provide evidence that the party has been notified and to ensure that the legal process can move forward. Proper service is crucial for the court to have jurisdiction over the parties involved.
